Treatment for Achilles tendinosis is similar to that for Achilles tendonitis but not same. Achilles tendinosis is chronic degeneration of Achilles tendon and treatment aims at arresting degeneration and promoting healthy growth of tendon tissue.
Treatment Options for Achilles Tendinosis
Platelet Rich Plasma Therapy: This is a new form of treatment. Your blood is collected and healing plasma extracted from it. This plasma is then injected into injured tendon triggering a healing process. Tendon tissue is resurrected and tendon heals without any major surgery.
Surgery: This removes abnormal tissue and accelerates healing. Surgery is not the foremost treatment option. By the time you go in for surgery, tendon condition deteriorates. Hence, surgery removes weakened tendon tissue and stitches together healthy parts of tendon tissue. If tendon deterioration is beyond this stage, tendon transfer is done.
Rest: This is of paramount importance in healing Achilles Tendinosis. Rest will help in reducing inflammation and will arrest the further deterioration.
Prevention: Treatment aims at impeding Achilles tendon pain and development. Orthotic devices like arch supports, heel cups, and other foot mechanics provide support to correct foot abnormalities and prevent further injuries to Achilles tendon.
Immobilization: If tendinosis is acute, immobilizing the leg is the best treatment. A cast or use of removable walking boot helps in subsiding inflammation extensively. However, immobilization should not be for too long, as stiffness would set in.
Ice Application: Apply ice over inflamed area to reduce pain and stimulate blood flow. Apply ice as many times as necessary preferably after exercise. Although this is not a specific treatment option for Achilles tendinosis, it inhibits discomfort due to inflammation.
Shoe Inserts: Place heel wedge inside shoe to lift heel by a small margin. This reduces significant pressure on tendon and accelerates recovery.
Exercises: Stretching and strengthening exercises improve flexibility of calf muscles. Increased blood flow reduces chronic pain in tendon region.
Medications for Achilles Tendinosis
Tendons take a long time to heal. Partial tear stimulates production of disorganized type-III collagen. Since this is weaker than tendon tissue, further injuries aggravate problem of tendinosis. Since this is not an inflammatory disorder, anti-inflammatory medications are not recommended.
